Do you understand and agree that if you were approved for adoption and for ANY reason are unable to keep the animal, you cannot sell, give away, or dispose of the animal. Rather, it MUST be returned to Blue HAWK? ______________________
PLEASE READ THE FOLLOWING STATEMENTS AND INITIAL TO INDICATE YOUR UNDERSTANDING & AGREEMENT TO TERMS OF ADOPTION THROUGH BLUE HAWK.
___ I agree animal will wear current ID at all times, including current rabies vaccination.
___ I agree animal will be current on all vaccinations and receive regular veterinarian care as needed.
___ I agree animal will be provided shelter, healthy food & fresh water at all times and be treated with love, attention and care.
___ I agree animal will never be constrained by a rope/chain, but will have an adequate size fence.
___ I agree animal will be an indoor pet &/or live in a country home.
___ I agree if I must leave animal for more than one day, they will be boarded in a kennel or provided care by a reliable caretaker.
___ I agree an agent of Blue HAWK has the legal right to home visits: if at such time it is found that I cannot/am not providing humane care of following the guidelines of this adoption contract, Blue HAWK has the legal right and moral responsibility to remove animal from my home and retain custody.
___ I understand that to the best of their ability, animals are monitored for behavior, health & personality by Blue HAWK, and that they will share any issues of which they are aware but are not responsible for conditions/behavior of which they had no knowledge.
___ I understand adoption donations help cover some of the cost of care the animal has received. This fee is considered a donation and as such is tax deductible, but non-refundable. The only exception is stated below.
___ I understand it is recommended I take animal to my veterinarian within 72 hours of adoption for a health check; if it is found at that time that animal is not of good health/has medical condition I may return animal to Blue HAWK for a full refund. A signed statement on clinic stationary describing condition/findings must be presented to Blue HAWK & donation will be refunded within 2 business days, giving Blue HAWK to confirm findings. Blue HAWK does not pay medical expenses incurred as Blue HAWK works with certain clinics for medical care- only adoption donations are refunded.
